I consent to allowing Friends of Fieldworkers to use any photos or videos taken of me for marketing and promotional purposes on its website, on social media pages, and in promotional brochures.
I hereby certify that I have no criminal record, and have never been accused or convicted of sexual abuse and/or assault.
